Your One-Page Household Emergency Sheet

Put the information a helper needs first on one page: your address, contact numbers, meeting places, and the location of essential supplies.

Open the free printable worksheet. It opens without an account. Use the Print button to print a blank copy or save it as a PDF, then complete it by hand or in a PDF editor you trust. The worksheet does not collect or send your household details.

Complete these fields

  1. Address and access: full street address, unit, and any relevant entrance instructions.
  2. Emergency number: the applicable number for your location. In the United States, use 911 for an immediate emergency.
  3. Household contacts: names, relationships, and phone numbers.
  4. Backup contact: someone who has agreed to help or relay messages.
  5. Meeting places: an agreed safe location near home and an alternative outside the immediate area.
  6. Essential locations: flashlight, first-aid kit, and the location of a separate care plan if needed.
  7. Review date: who checked the information and when.

Use a simple example

Here is a fictional contact entry:

Backup contact: Morgan Lee, family friend, 202-555-0168. Morgan has agreed to relay messages if household members cannot reach each other.

A location entry should be just as clear:

Flashlight: labelled drawer beside the front entry. Spare batteries are in the same drawer.

Use specific details that the intended reader can recognize. “Call Mum” or “look in the usual place” may leave a caregiver unsure what to do.

Choose what belongs elsewhere

Keep passwords, PINs, recovery codes, identification copies, and detailed financial records in separate secure storage. Decide whether health details belong on this shared sheet or in a private care plan available to the appropriate person.

The sheet is a quick reference. It does not replace instructions from emergency services or a clinician-provided treatment plan.

Check the finished page

Ask a trusted adult to locate your address and backup number. Walk together to the agreed meeting place. Check that it is appropriate for the hazards relevant to your area and identify an alternative if it cannot be reached safely.

Store the sheet where its intended users can find it. Keep a spare with your emergency supplies if appropriate. Choose a review date and replace copies after a contact, address, or household need changes.

Add more only when you need it

The emergency binder checklist covers fuller records. If a guided manual would save you work, inspect the Hardcopy sample pages and check the plan options.

Put your household details into a manual

Hardcopy guides you through the information, then prepares a household emergency manual. You supply the details and check the result.
